Housing Transition Navigation Services (HTNS)
We help Medi-Cal members:
Find and apply for housing
Complete rental applications and secure documentation (ID, proof of income, etc.)
Develop a personalized Housing Support Plan
Connect with housing resources like Coordinated Entry and HUD programs
Who qualifies: Medi-Cal members at risk of or experiencing homelessness with behavioral health needs, chronic conditions, or disabilities.
Housing Tenancy and Sustaining Services (HTSS)
We help Medi-Cal members:
Receive up to 6 months of rental assistance during a housing transition
Secure short-term housing while moving out of:
Institutions (e.g., hospitals, jails, mental health facilities)
Congregate settings (e.g., shelters, temporary housing)
Unsheltered homelessness
Coordinate rent payments with landlords, housing providers, and Managed Care Plans
Maintain stable housing while pursuing long-term solutions like permanent housing, employment, or medical treatment.
Who qualifies: Medi-Cal members who are experiencing or at risk of homelessness, have complex health needs such as behavioral health conditions, chronic illnesses, disabilities, or are pregnant/postpartum, and are transitioning out of institutional, carceral, or temporary living settings.
Transitional Rent Coordination
We support individuals in staying housed by:
Educating on lease responsibilities and tenant rights
Coaching on communication with landlords or neighbors
Helping prevent evictions or lease violations
Providing regular wellness check-ins and housing plan updates
Who qualifies: Medi-Cal members who have recently obtained housing or need extra support to maintain it.
