At Haven Path, we believe stable housing is a powerful form of healthcare—and we can’t do this work alone. That’s why we actively seek partnerships with organizations that share our mission to support Medi-Cal members in securing and sustaining safe, stable housing.

We deliver Housing Transition Navigation Services (HTNS), Housing Tenancy and Sustaining Services (HTSS), and Transitional Rent coordination under California’s CalAIM Community Supports initiative. If your organization touches health, housing, or human services—we’re likely natural collaborators.

🩺 Medi-Cal Managed Care Plans (MCPs)

We partner with MCPs to provide reimbursable, high-quality housing support services that align with DHCS requirements and improve member outcomes.

👥 Enhanced Care Management (ECM) Providers

We support ECM teams by helping their members navigate housing systems, secure safe units, and stay stably housed.

🏘️ Housing Authorities & Coordinated Entry Systems

We collaborate with local CES leads, Continuums of Care (CoCs), and housing authorities to ensure referrals and resource alignment.

🧠 Behavioral Health & Reentry Providers

Many of the individuals we serve are exiting institutional care or living with behavioral health challenges. We welcome coordination with reentry services, SUD programs, and mental health teams.

🏥 Health Systems & Hospitals

We’re a trusted community-based partner for hospital discharge planners and care coordinators looking to connect patients with housing support.

🌱 Community-Based Organizations (CBOs)

From legal aid to food assistance, we work best when members are wrapped in a network of supportive services. We value mutual referrals and co-enrollment models.
