    • We conduct a comprehensive housing assessment to identify each person’s goals, needs, and barriers.

    • We develop a personalized Housing Support Plan to guide their path toward housing stability.

    🔍 Housing Search & Application Support

    • We assist members in searching for available housing that fits their preferences and eligibility.

    • We help complete rental applications and gather required documentation such as ID, Social Security card, and proof of income.

    💼 Benefits Navigation

    • We support members in accessing public benefits like SSI, CalWORKs, and other programs that contribute to long-term stability.

    🤝 Landlord Coordination

    • We engage with landlords on behalf of the member, advocate for housing access, and educate property owners about housing support programs.

    🚚 Move-In Support (Non-Financial)

    • We coordinate transportation and logistics to support a smooth move-in process.

    • We ensure the unit is safe, clean, and ready for the member to call home.

    🧾 Tenant Education

    • We educate members on their Fair Housing rights, how to request reasonable accommodations, and how to maintain a successful tenancy.

    Note: Under Housing Transition Navigation Services (HTNS), we do not pay rent or deposits—but we coordinate all the steps needed to help members secure housing.

  • At Haven Path, we support Medi-Cal members in maintaining stable housing through consistent, personalized tenancy services. Based on each member’s needs, we offer:

    🧾 Tenancy Coaching

    • We educate members on their lease obligations and rights as tenants.

    • We provide support with household management, including basic budgeting and independent living skills.

    🤝 Landlord/Neighbor Mediation

    • We help members resolve conflicts with landlords, neighbors, or other tenants.

    • We coordinate with landlords and property managers to maintain positive relationships and prevent misunderstandings.

    🚨 Eviction Prevention

    • We identify early warning signs of lease violations or housing instability, such as late rent or complaints.

    • We work with members to develop crisis prevention plans and connect them to appropriate resources.

    💼 Benefits Support

    • We assist with annual housing recertifications, income updates, or changes that affect tenancy.

    • We help ensure members maintain access to housing subsidies and public benefits.

    🏠 Home Visits & Unit Safety

    • We conduct regular check-ins to confirm that the living space remains safe, clean, and habitable.

    • We identify any maintenance needs or safety concerns that may put the tenancy at risk.

    📄 Plan Maintenance

    • We update the member’s Housing Support Plan as their needs evolve to ensure ongoing support and long-term stability.

    🌐 Community Resource Linkages

    • We connect members to services like food assistance, transportation, mental health care, and life skills programs that help support their success in housing.

  • 📋 Eligibility Verification & Coordination

    • We verify that each member meets the clinical and housing-related eligibility criteria for Transitional Rent under CalAIM.

    • We coordinate with Managed Care Plans (MCPs), Enhanced Care Management (ECM) providers, and housing partners to initiate and sustain the benefit.

    🏠 Housing Support Plan Management

    • We develop a detailed Housing Support Plan that outlines housing goals, transition timelines, and supportive services.

    • We update the plan regularly to reflect progress, barriers, and changes in housing status.

    💵 Rent Benefit Coordination

    • We collect and manage required documentation, including lease agreements, move-in confirmations, and payment authorizations.

    • We work with landlords, housing authorities, and MCPs to ensure rent payments are tracked and processed accurately.

    📊 Utilization Tracking & Compliance

    • We monitor the duration of the rent benefit to ensure it does not exceed the 6-month limit within the 5-year demonstration period.

    • We maintain records in line with MCP and DHCS documentation standards to support reimbursement and compliance.

    🔗 Service Integration & Transition Support

    • We pair Transitional Rent with HTNS and HTSS services to promote long-term stability.

    • We help members transition to permanent housing resources, such as Rapid Rehousing, Permanent Supportive Housing, or vouchers.
